The first acupuncture appointment is 90 mins, to allow time for a full discussion of your health history and goals. The following forms are required for all new patients, many patients find it convenient to fill it out ahead of time. You may email them ahead of time to nicholesantoro.lac@gmail.com or bring the completed forms to your first visit. You can also fill it out at the office prior to your initial intake. Remember to also bring your ID and your most current insurance card.
